Passing Value From A Created Data

I have a form called Form-1a and a Home Visit

First is I create Form-1a when i save the form1-a with details and everything, when i view the form-1a there will be a link to the side that will lead it to create the home visit form ... what i want to do is when i click the link some of the values from the created form1-a will be a default value to the home visit form ...

i am a newbie and don;t have any idea how to do that ...
